Dear Parents,
This year the Brophy Community Foundation has advanced its financial aid process by going online. Financial aid applications will be available online starting January 28, 2008. Applying online is the fastest and most direct method of submitting your application. We greatly recommend using the online application services.
Please use the following instructions to process your application:
- Please click here to get the FACTS web site: www.factstuitionaid.com.
- Select “applicant sign in”
- New users select “Create an Account” and setup “user name” and “password” (please write down and keep in safe place). OR Returning users enter your existing “user name” and “password”.
- Select school using the school search zip code.
- Select the school your child is attending.
- Select “continue”.
Once you have registered and signed in, you will have the ability to complete the FACTS Grant & Aid Assessment application online.
After completing the online application you will need to mail or fax the supporting documentation to FACTS Grant & Aid Assessment; PO Box 5688; Lincoln, NE 68505-0688. The following tax information is needed to verify your application:
- Signed and completed paper application or completed online application
- Copies of your 2007 tax forms including all supporting tax schedules.
- Copies of your 2007 W-2 form for both you and your spouse.
- Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, or any other miscellaneous income.
Please do not submit multiple applications. Please note: Faxed applications will not be accepted.
If you do not have access to the internet or need the application in Spanish, paper applications may be mailed to you by calling the Brophy Community Foundation at 602-264-5291 x.6500. Spanish applications are not available online.
If you have any questions or concerns about the application process, you may speak with a FACTS Customer Care Representative at 1-866-315-9262.
REMEMBER: Parents must re-apply every year for financial aid.
